That pic is of a lawn mower transaxle.
Commonly made by peerless.
If that is what you want, find a cheap wrecked up Lawn tractor on Craigs list. and steal the transaxle.
Shifter kart engines are a transmission/engine unit.
You cannot simply add a shifter kart transmission to a 13hp Honda.
Here is a great example of a shifter kart engine:So i would have to get a engine (like a car engine) then put a chain sprocket to the end?

No, a motorcycle/dirt bike or a big twin trans with an added reverse gear(they make these for trikes. But these are serious money)
What about could he use a Small dirt bike or quad engine. 125cc or 200cc.
Those have the tranny built in??? and some of the Quad ones have reverse?
Nope My 86 and 87 Honda TRX125 both have reverse. and No clutch manual transmissions
Honda TRX250 is the only manual ATV I know of that has reverse.
Even a semi automatic would work, basically anything under $150-$200
