Set screw goes in crooked

On my clutch, the beginning threads are off so the set screw goes in an angle and then stops. Has this happened to anyone before? Can I just file and smooth out first few threads?

Its a manufacturing error... If you can I would run a tap through to clean up the threads... Youll have to check what thread it is first.
