He means has the cable sheath slipped in the attachment points? If this happens, it effectively makes the top of your pedal travel dead motion, and it doesn't pull the throttle fully open.
This can happen quite easily if your pedal travel potential is longer than your throttle arm travel, and the throttle arm is acting as the pedal stop via the cable.
To avoid this happening, your pedal stop should be set so it touches at precisely the same point as the throttle arm touches the stop screw.
