I've had myself, and customers with this issue. It's ez, cost hardly nothing, and could save a trip to the emergency room.
It's jus a simple lock for the bolt to hold the driver on, and no worries of it flying off.
I use a 5/16" i.d. fender washer. Put it in a vise, use a cut off wheel, make a small narrow tab to fit, bend inside the cut out for the key way. Then make 4 or 5 more wider tabs to bend (1) back over the crankshaft bolt. Jus make sure you catch at least 1 flat edge of the bolt head, to keep it from turning, backing out.
To unlock it, jus pry it up with a stiff screwdriver, or small chisel. If it breaks the tab, while prying it off, jus use 1 of the extra tabs you already made earlier.
I find that hardened fender washers snap easy, the cheaper metal washer, in my hand, seem to work best and can reuse a tab a few times before it snaps.
I recommend bending the extra tabs over against the driver, until needed, installing the cover to keep little curious fingers away from the SAW BLADE looking lock.
I've been installing this simple lock for over 3 yrs now, on countless 30 series drivers, and have had no one come back and say it came loose, or flew off. Usually, they come by and I take it off for cleaning, maintance, belt changes or whatever.
It's simple, it's cheap, it works, and most important...... it's safer.
