Cut off the flange of the old one. Wire brush(or sand/grind ect.) the crap out of it, till it gleams bare steel.
Get some pipe, EMT or other wise steel tube. Pick a good length for your aplication, weld together. Also weld on a female pipe fitting for the mating of a Briggs style screw on muffler. Good to go.

Now this is a general description, it is also depends on how enthusiastic you are in building it. Your location, will help to source various of pieces. or could just scrounge for the bits you need.

The level of quietness is in ones perspective. As per decibel level, it varies as to the design.

More importantly what is this for? Vehicle and aplication, I dont think that it was said?

Well what has been said about making your own muffler is the cheapest and acceptable way to go about it. However it will not be quieter than the stock muffler. What is being made by taking the old flange and putting a pipe on it makes a straight pipe. This give the engine a bit more power at the cost of noise reduction and emissions.
The stock muffler, if it is still good, would be a much better option for quiet.

How do I check if the muffler is damaged or broken?
You can see if the muffler is damaged by taking it off of the engine and looking at it with your **** eyes. I'm sorry some questions are not to be asked but researched as it is very hard to explain to you if I cant see the muffler. If you wish to continue to ask questions I recommend taking off the parts in question and showing us detailed pictures with good lighting so we can point out things to you.

Also Can I clean the air filter or does it always have to be replaced
You can clean the air filter if it is not wrecked. You will know if it is wrecked it wont look very good. You can clean the filter with water and absorb the oils and water off with a rag gently pressing the filter and not wringing it into the rag.

Hope this helps do as much research on your own as you can you cant always get everywhere in life expediently by asking strangers questions. A good place to start researching is Youtube I used to watch this guy called Donyboy73 who did small engine repair videos and helped me repair a lot of our power equipment and I pretty much learned all I know about small engines from the internet. Put in the effort on that motor and You will soon be able to answer your own questions and other peoples questions. Also you will stare at your motor for hours with pride whispering to it, "I've been inside you".

Tell you the truth i got an complete go kart at a yard sale for 75 dollars and it was a simple yard kart with one of these on it. The motor was fine but just need some cleaning of the carb an some clean gas but it ran fine but the carb was still junk from where it sat for 1 or 2 years so we just ordered a new on and it ran fine. it suprised me when i got on the gas.

Sorry, what I meant to say was I already have a electric motor, but I am converting it to gas. If I could get a clutch for a #25 I won't have to buy a new chain and a sprocket for my axle.

you should not run an gas engine with a #25 chain.

well not if you have more than 49cc displacement OR more than 6k rpm!

anything smaller can get really dangerous really fast;
if you intend to get it off the roads you should consider a #40 chain already...
