Indeed it does look cracked, good eye! I'd pull the engine and see how bad the plate under it is cracked, very well could be part of the reason it throws the chain constantly.

Driver pulley is definitely installed backwards and is likely most of your issue, hard to tell if it's a 30 or 40 series, but I'm guessing it's a 30 series based on the backplate you have installed...

For as little as $60 I'd go with a t/c, you'll probably need to buy the bolt for the end of the shaft because they don't typically send the right one.
