It does look like the engine is canted towards the left slightly. That mounting plate is a bit on the sketch side if it's just held down with long bolts.

Also check the valve clearance.

$5 set of feeler gauges from the auto parts store will do both, if you don't have a set already.

Try adding a center bearing? Axle flex puts extra slack in your chain and throws off the sprocket alignment.

With 23" tires I don't think you want as tall a ratio as 5 or 6. In a vehicle that size, you'd probably end up needing to put a new belt on it every time you fill it with gas if you're running it...
